Here's CSX SD-10-2 number 8888.

The story goes that while switching in the yard in Ohio, the engineer hops out of the cab to line a switch that was set wrong. He set the independent brake on the locomotive and put the throttle up to full power, but because the 47 cars be pulled were not charged with air, he knew it wasn't going to stop and as soon as he got off the engine took off on to the main at full speed and burned the brakes off the lead engine. The runaway, carrying lumber, autos, coal, grain and with tank cars carry deadly chemicals speed up to 51 mph though towns and over crossings with no one at the controls. It wasn't until a second CSX locomotive catches up to the runaway in reverse, manages to slow down using dynamic braking up to 11 mph and an employee manages to hop on and shuts the thing down bringing it to a stop.
